‘The Transition Scoping Project. Adolescents and Young Adults supported by Children’s Hospices: A project for understanding needs and current provision.’
Post: Project Assistant Hours: Full time Term of consultancy: Up to 6 months
Salary: £10,000 paid in monthly instalments for 6 months
Working base: To work from home with access to hot desk space at the children’s hospices Christopher’s (Guildford, Surrey) and Shooting Star (Hampton, Middlesex).
Background to the scoping project:
Currently CHASE and Shooting Star recognise the pressing need to understand more about:
The size of the population in the geographic areas they support of young people who will need to transfer from children’s hospices to adult services.
The services already existing in their communities that offer support to this age group and identify gaps in provision.
What guidance is needed for the staff who support the young people through the period of transition as well as meeting the developmental needs of those who will die in adolescence.

This approach has not only built our profile across SW London, Surrey and Sussex. It’s won us national recognition too. We have ambitious plans to expand our services and reach out to even more families. This will require even more funds to be raised through charitable donations. To achieve this we are expanding our fundraising team and need to recruit experienced staff, all based at the CHASE Fundraising Office in Guildford, Surrey and reporting to the Head of Fundraising.
For this post you will be expected to:
Lead the development of major giving strategies to maximise sustainable income generation for CHASE.
Develop, implement and manage an operational plan for increased giving via high net worth individuals, trust and statutory income sources.
Research, plan and develop specific high net worth individual and trust and statutory relationships, maximising opportunities to work in partnership with key individuals; engaging as appropriate senior CHASE personnel to enhance the relationships.
Ensure the collation of all information and preparation of targeted and persuasive applications for approval and submission to trust and statutory income sources.
Motivate, support and develop individuals within the High Level Giving team.

Summary of post
There are many children/young people who live day-to-day with conditions that mean they probably won’t reach their 19th birthday. Here at CHASE, we’re dedicated to supporting the lives of those children and their families. But it’s how we do it that sets us apart. Laughter, fun and new experiences are key to our special kind of care. To us, it’s all about making every day count, not counting every day.
The post holder will work as a member of the Therapy Team, as well as a fully participative member of the multi professional care teams at Christopher’s and in the Community. You will be able to work on your own initiative and as a team member. You will be expected to initiate as well as implement existing therapy plans, monitor and adapt these in liaison with the parents and the statutory therapy service provider.
You will be actively involved in the education, training and development of other team members as well as in disseminating and maintaining therapy programmes delivered by other care team members and undertaking appropriate training sessions.
Experience of working with children is essential. Knowledge of the equipment needs of children with disabilities and in providing assessments for home adaptations is desirable as is training as a back care advisor.
The CHASE service is provided 24 hours a day, 365 days a year and the post holder will be expected to work flexibly to meet the needs of the children and family.
